Project LUCID Post-Release Survey


This is a survey from Project LUCID, a community-based action research project funded and supported by the City of Toronto and Laidlaw Foundation. We are not part of the Ministry of Community Safety & Correctional Services (MCSCS) or the adult (18+) provincial correctional facilities (e.g. jails, detention centres, correctional centres) they manage.  

At this time, we are looking to understand the current program/service landscape inside and outside adult provincial correctional facilities in and around the Greater Toronto Area (GTA). We will use the information we collect to develop recommendations for government and service providers on how to improve programs and services for people who are or have been incarcerated.

This survey focuses on (1) your experiences with programs, services, and supports at the adult provincial correctional facility you were incarcerated in before your recent release, and (2) your experiences with programs, services, and supports in the community since your recent release. (For project purposes, recent release means released from provincial custody within the last 12 months).

Your participation in this survey is voluntary. Your decision to participate, or not participate, will not have any effect on probation, parole, or ongoing court proceedings (e.g. sentencing or trial outcomes). There are minimal risks associated with your participation. However, risk in the form of stress or anxiety may be more likely if you are uncomfortable thinking or answering questions about your experiences with programs, services, or supports. If you experience discomfort, you can contact St. Stephen’s Community House at (416) 925-2103 for help in obtaining support. You can also skip any questions that you are uncomfortable answering and/or discontinue participation at any time, without consequence. Once you submit the survey you will no longer be able to withdraw. The information collected in this survey is anonymous. This means that any information you provide in this survey will not be linked to you. Information collected from this survey will be stored in a password protected electronic format, and will only be accessed by the research team.

This survey will take about 20-30 minutes of your time. When you submit the online survey, you will receive a confirmation code. This code will be used to provide you with a $15 gift card for your participation. Please record this code and give it to the survey facilitator/service provider helping you with the survey. If you complete the survey without assistance from a facilitator/service provider, please contact the research coordinator with your confirmation code to make arrangements for the gift card.
